If you have a bad feeling it may be more likely to be anxiety than positive excitement. My guess is that the probability of this being a medical problem is small. If you're concerned, you could always ask your doctor about it. My guess would be that in most cases it's likely that there may be some stressful thoughts / fears / anxieties going on in your head which are triggering the butterflies.
